A rail disaster in Spain may have been caused by a combination of system failure and human error. Preliminary hypotheses suggest a traffic controller failed to warn an AVE train driver about another train on the tracks. The second train was stationary for unknown reasons about 25 kilometers ahead. This lack of communication led to the collision.
