Adif opened an incident report at 8:30 on Wednesday. This report was filed 23 minutes before the accident occurred. The filing notes that the LZB railway security system in Urda failed. This failure was caused by a series of technical issues. These problems included losses of voltage and failed telephone communications. The system also experienced intermittent loss of information.
